Fran Bishop from the Adults Social work team attended the April ChAD meeting to ask the committee members their views on the proposed communication passport The passport is a document to help people to hold the correct information about people to help them engage more effectively with individuals.

 

Fran asked if ChAD could design a logo to add onto the top of the form so it has an identity.

Since this meeting, the committee suggested ideas collectively and several members have since competed the task. The logo has been forwarded to the relevant parties.
